9:30-10:20a.m. Workshop # 1: Care Mapping Your Loved One’s Needs: How to identify
and gauge the level of services necessary to support your loved one’s journey
This session introduces the concept of indicators and how they are used to care-map
the level of care and services that one could need. It is designed to answer the questions
of “When is it time to consider additional services?” “When do I need to consider Assisted
Living or Nursing Home placement”. This session offers concrete definitions on what each
level of Long-term Care services provides. Presented by: Carlette McDaniels, LCSW

11:10-11:50a.m. Workshop # 2: Planning with Compassion: Hospice Care, Palliative
Care, and Advance Directives
Join us for an informative panel discussion designed to help family caregivers better
understand hospice care, palliative care, and the importance of advance directives in
end-of-life planning. Our panel of professionals will explain how palliative care can
support comfort and quality of life at any stage of serious illness, while hospice care
focuses on compassionate care during the final stages of life. Caregivers will also learn
how advance directives—such as living wills and healthcare proxies—help ensure a
loved one’s wishes are known and respected. This session will provide practical
guidance and compassionate insight to help families plan ahead with confidence.

11:50a.m.-12:30p.m. Closing: Caregiving by the Commandments: Practical Support
and New York State Resources
Caregiving is deeply meaningful work, but it can also feel overwhelming and isolating.
This session brings inspiration and practical support together by using the Ten
Commandments of Caregiving as a framework to explore essential principles that help
caregivers sustain their strength, compassion, and resilience. New York State programs,
services and supports available will be highlighted throughout this session.
